All services for Botox and fillers require a consultation first and pricing will be determined due
to the various areas needed for treatment. Photographs will be taken before and 2 weeks after
treatment. Clients will be educated on before and after care instructions. Consent forms will be
completed the day of your consultation. Clients aging from 21 to 55 years of age will be given a
complimentary pregnancy test before any Botox or filler treatment. If you refuse to take a
pregnancy test, then you can sign a consent form to wavie the pregnancy test.
What is Botox?
Botox is a cosmetic prescription used for a multitude of cosmetic concerns. It's FDA-approved
for both aesthetic and therapeutic treatments. It's a temporarily treatment that can be given
once every 3 months. It is recommended for frown lines, forehead lines, eyebrow lifting,
bunny lines (nose), and crow's feet lines (eyes). There are other sites on the face that can
benefit from Botox and can be discussed further during your consultation. Botox works
overall by sending signals from your nerves which will then cause your muscles to contract
in the targeted area. Botox is recommended for patients 18-70+ years old. The treatment
is quick and has minimal downtime. Aftercare instructions will be discussed and given

(Always discuss stopping any medications with your primary doctor prior

Avoid alcohol 48 hours before your appointment
Avoid caffeine at least 24 hours before Botox
Eat something the morning of your appointment

What are Fillers?
Versa by Revanesse and Lips+ are FDA-approved hyaluronic acid (HA) dermal fillers used for
the purpose to treat submucosal implantation for your lips, and areas within the face that
appear hallow or just lacking its fullness. HA is a natural component of the skin which gives
a refreshing and younger appearance. It is intended for patients 22 years of age or older.
Numbing cream will be provided to patients for this treatment because of the depth
needed to have a positive outcome.

No alcohol 24 hours prior to appointment
